If you want to process your account online, our practice requires your email address and mobile number. This information is used by Infomedics to give you secure access to the online portal where your account is ready. If you log in to your Infomedics portal, enter the SMS code you receive from Infomedics. If you prefer to receive your account on paper, you can set it on your portal.
